    Can Your Diet Protect You From Dementia?

    Can Diet Reduce Dementia Risk?

    We often talk about food as fuel for the body. But what if it’s also medicine for the brain? This may particularly be the base if you have some metabolic dysfunction.

    new study from the UK Biobank, published in 2024, suggests that what you eat could make a meaningful difference to your dementia risk, especially if you have type 2 diabetes, heart disease, or a history of stroke. The researchers tracked over 84,000 people aged 60 and up for more than a decade. They examined their diet, health conditions, and in some cases, their brain scans.

    The Inflammation Connection

    The link between inflammation and disease certainly isn’t new. We know that chronic low-grade inflammation plays a role in heart disease, type 2 diabetes, and even dementia. The term “inflammaging” has been coined to describe this slow-burning, systemic irritation that seems to drive many age-related conditions.

    The Dietary Inflammatory Index (DII) ranks your diet based on how likely your diet is to promote inflammation. Pro-inflammatory diets are heavy in processed meat, refined carbs, processed foods and added sugars. Anti-inflammatory diets tend to be rich in vegetables, fruit, whole grains, fish, legumes and olive oil. Essentially, a Mediterranean-ish pattern wins again.

    The Main Question

    The researchers wanted to know whether an anti-inflammatory diet reduces dementia risk. This was especially pertinent for people already at higher risk due to cardiometabolic disease (CMD). CMD in this study included type 2 diabetes, heart disease, or stroke. This is because people with CMD are at higher risk of dementia. 

    In answer, the anti-inflammatory diet does reduce dementia risk, and it’s not a trivial difference.

    Compared to people without CMD who ate an anti-inflammatory diet (the “healthiest” group), those with CMD and a proinflammatory diet were more than twice as likely to develop dementia. But those with CMD who ate an anti-inflammatory diet had a 31% lower risk of dementia than their proinflammatory peers.

    They also developed dementia two years later, on average.

    Brains on Scan

    Around 9,000 people in the study had brain MRI scans. This allowed researchers to look for changes associated with cognitive decline.

    People with CMD and a proinflammatory diet had smaller grey matter volume (the brain cells you want to keep) and more white matter hyperintensities (a marker of small vessel disease and vascular injury). Those with CMD and an anti-inflammatory diet had significantly better brain structure. This was reflected by more grey matter and less white matter damage. Not perfect, but certainly better.

    A Few Caveats

    Like any good study, this one has its limitations. Diet was assessed using 24-hour food recall, which isn’t perfect. People might forget what they ate, or fudge it slightly to sound healthier. Also, the DII is based on nutrients rather than food types. So, it can’t distinguish between plant-based and animal-based proteins, or account for cooking methods that might influence the inflammatory load of a meal.

    Still, participants with higher DII scores had higher levels of inflammatory markers in their blood. This lends credence to the idea that the DII is capturing something meaningful.

    The Takeaway – avoid the takeaway

    If you’ve already got diabetes, heart disease or a history of stroke, your brain may be more vulnerable. But your diet might offer a way to fight back.

    Choosing anti-inflammatory foods won’t reverse existing brain damage, but it might slow the progression of cognitive decline. At the very least, it’s a low-risk strategy. It also benefits your cardiovascular health, blood pressure, gut, and energy levels. 

    If your goal is to keep your brain sharp, go for plenty of colour on the plate. A good example might be salmon and lentils over sausage rolls and white bread. It’s not about perfection. It’s more about shifting the odds in your favour.

    And in this case, the odds are worth shifting.

    Multivitamins and Memory: The COSMOS Study

    The debate around multivitamins (MVs) has been as divisive as pineapple on pizza. Now, I believe that pineapple on pizza is fundamentally wrong. I am also of the belief that nutrients derived from whole foods are likely better than does derived from pharmaceutical companies. However, to repeat a quote often attributed to John Maynard Keynes “When the facts change, I change my mind. What do you do, sir?” Enter the COSMOS trial, stepping into the fray like a peacekeeper, armed with data rather than diplomacy. The study’s latest findings suggest that daily multivitamin-mineral (MVM) supplementation might be the secret sauce to preserving our cognitive zest as we age. But was it enough to change my mind on multivitamins?

    From The Farm to The Pharmacy

    An ideal multivitamin is derived from whole foods. These not only deliver vitamins and minerals but also fibre that nourishes our microbiome and slows our absorption of sugar. The Iowa Women’s Health Study suggested that regular MVIs might actually increase mortality in older women. This brings us to the COSMOS trial. The initial COSMOS trial was an investigation into whether Centrum Silver or a supplement of cocoa and green tea flavonoids, could improve health outcomes.

    COSMOS: A Universe of Data

    Sponsored by the titans of industry, Pfizer and Mars Edge, the COSMOS study lasted 3.6-years and included 21,000 participants. The cocoa and green tea flavonoids were found to reduce cardiovascular mortality by 27%. The spotlight then turned to MVIs and their role in our cognitive health. In the later COSMOS-Mind paper, there was no cognitive protection with the consumption of flavonoids. However, there was a trend towards a lower incidence of mild cognitive impairment and dementia in the MVI group.

    This latest study featured an ensemble of subsets from the COSMOS trial. There participants had been assessed for cognitive changes over two years. It found that those on the MVI regimen showed modest improvements in global cognitive function and memory. This effect suggested a slowing of cognitive ageing by about two years. A modest but significant victory in the battle against time.

    The Ingredient List: A Culinary Adventure

    Examine the ingredient list of an MVI you’ll find a melange of fat-soluble vitamins, the B’s and C’s of the vitamin world, and a mineral ensemble. The COSMOS MVI (Centrum Silver) also boasted lutein, which can be beneficial for eye health and lycopene, a constituent of tomatoes that has been linked to lower blood pressure.

    To Pop or Not to Pop?

    So, should we turn to MVIs as our cognitive shields against the ravages of time? While the risk-benefit ratio of MVIs appears favourable, one can’t help but wonder if we’d be better off investing in a cornucopia of fruits and vegetables. I would prefer supporting our local farmers rather than the pharmaceutical giants.

    A Plant-Forward Path to Cognitive Health

    In the end, the COSMOS trial adds a few more threads to the ever-expanding tapestry of health research. It whispers, rather than shouts, the potential benefits of MVIs on our cognitive well-being. Perhaps the true takeaway is a reminder of the holistic approach to health. A diet rich in natural vitamins and regular physical activity might just be the recipe for a vibrant mind and body. Where a multivitamin is likely to be of benefit is for those people who are unable to access fresh or frozen fruit and vegetables.

    The best multivitamin might just be a sun-ripened tomato from your greenhouse (or Bayley and Sage in my case) or a crisp apple from the local orchard (or Fulham Greens). In the quest for cognitive longevity, it’s not about the pills we pop, but the foods we savour and the lifestyles we embrace.

    Everything you need to know about genetic testing for Alzheimer’s disease

    TV presenter and journalist, Fiona Phillips, recently shared her Alzheimer’s disease diagnosis at the age of 61. Fiona has been an ambassador for Alzheimer’s Society and has undergone genetic testing for Alzheimer’s due to her family’s history with the disease (both her mother & father had Alzheimer’s). 

    Since Fiona shared her diagnosis, many of our patients have been in touch with questions, particularly those with a family history of Alzheimer’s disease and dementia. In this latest article we want to share everything you need to know about genetic testing for Alzheimer’s disease:

    What is Alzheimer’s disease?

    Alzheimer’s disease is a physical illness which damages a person’s brain, eventually leading to dementia. Alzheimer’s disease is the most common cause of dementia, which describes a set of symptoms that over time can affect memory, problem-solving, language and behaviour. Alzheimer’s disease is the most common type of dementia.

    In Alzheimer’s disease ‘plaques’ composed of proteins (called amyloid and tau) build-up in the brain, making it harder for the brain to function properly. Often there are other ‘vascular’ factors that cause harm to the brain. These could be high cholesterol lipoproteins as well as high blood pressure.

    What are the causes of Alzheimer’s disease?

    Alzheimer’s disease is the most common cause of ‘dementia’. The many causes of dementia all cause difficulties in memory, problem-solving, and thinking. Symptoms can be subtle at the start. It may take years before they are severe enough to cause a problem in your day-to-day life.

    I have heard of a blood test to check for the risk of Alzheimer’s disease. What is the test?

    Last year Chris Hemsworth revealed he had a high risk of Alzheimer’s disease after a genetic APOE test.

    Testing for the gene changes that have been linked to early-onset Alzheimer’s might be useful for someone with a family history of the disease or for someone showing symptoms of early-onset disease. At Coyne Medical we offer APOE gene testing, in the form of a blood test, as an add-on to our health and genetic screening services.     What is the APOE gene?  

    APOE is a gene located on chromosome 19. There are three variations of APOE you can inherit, called ‘alleles’, epsilon (ε) 2, 3 and 4. You inherit any two combinations of ε2, ε3 or ε4, as you get one copy from each parent. For example your APOE genes could be ε2/ε2, or ε2/ε4 . ε4 is the variant which carries an increased risk. If you inherit two copies of ε4 your risk of Alzheimer’s dementia is 8 to 12 times the risk of someone with two copies of ε2.

    APOE genes alone won’t guarantee someone gets Alzheimer’s disease. Lots of other factors will influence this. Blood pressure, diet, cholesterol and physical activity levels all have an impact.

    About 1% of cases develop dementia due to an inherited genetic mutation in a single gene. One example is the amyloid precursor protein gene (APP). There may be a family history of dementia starting at a very early age, from 30 onwards. In these cases, it is important to talk to your doctor and consider your options for testing.

    Don’t Neglect Your Blood Pressure: Safeguard Your Brain Health

    Life is busy. Many of my patients are so busy that they, to paraphrase Gustav Stresemann, are walking “on the edge of a volcano.” It just takes one small event to teeter over the brink. When life is this hectic, it can be easy to neglect something that isn’t causing you any symptoms. Blood pressure is just one of those things. They don’t call it the “silent killer” for nothing. High blood pressure stalks many of us from middle age without us knowing, waiting for an opportunity to push us over the edge. 

    Ignoring your blood pressure in early middle age can have a compound adverse effect on your cardiovascular health. It is a bit like having debt on a credit card that you never get around to paying off but keep spending on in order to accrue the Air Miles. 

    High blood pressure in middle age can have a particular impact on brain health in older age. Almost half of the burden of Alzheimer’s disease and related dementia is due to factors that are within our power to change. High blood pressure is one of the most common of these factors. 

    recent study conducted by researchers at the University of California has shed more light on the connection between blood pressure and brain health. Their findings underscore the importance of monitoring and managing blood pressure, especially in early adulthood. 

    The study followed a diverse group of participants and found that individuals with hypertension (high blood pressure) or those who transitioned to hypertension in early adulthood had smaller brain volumes and poorer white matter integrity in late life. Interestingly, the negative impact of hypertension on brain volumes was more pronounced in men compared to women.

    Previous research has already indicated a link between hypertension and neurodegenerative conditions such as dementia. This recent study reinforces those findings and highlights the importance of considering hypertension earlier in life, even during early adulthood. Early adulthood hypertension and increases in blood pressure were associated with differences in brain volumes and white matter integrity, which are key indicators of cognitive decline and dementia.

    Notably, the study used updated guidelines for diagnosing hypertension, taking into account lower thresholds for diagnosis. This suggests that even individuals previously classified as having prehypertension or normal blood pressure could still be at risk for neurodegenerative changes associated with dementia.

    The study also uncovered some interesting sex differences. Hormonal factors appear to play a role in protecting women from the detrimental effects of hypertension on the brain, particularly before menopause. Men, on the other hand, seem to experience a stronger negative association between hypertension and brain volumes. The findings of this study emphasise the critical role of managing blood pressure throughout life, starting from early adulthood.

    It’s vital not to overlook the significance of blood pressure management. Just as neglecting credit card debt can result in mounting interest and financial distress, ignoring blood pressure can have a compounding effect on brain health. By regularly monitoring and managing blood pressure, particularly in middle age and early adulthood, you can take proactive steps to protect your brain health and reduce the risk of cognitive decline in later years. Consult with your doctor, make lifestyle changes, and adhere to any prescribed medications to keep your blood pressure in check.
