📖 Immunisation Records

Please bring your child’s previous immunisation records to the appointment. In the UK, this is often called the ‘Red Book’.

👨‍👩‍👦 Parental Permission

If your child is attending the appointment with someone who does not have parental responsibility (e.g., nanny, childminder, or grandparent), please provide them with a signed note giving your written permission for the specific vaccine.

⚠️ Important: Chickenpox Vaccination

We routinely offer the chickenpox (varicella) vaccine, which is a safe and effective way to prevent infection.

💡 Please note:
✅ There MUST be at least a 4-week gap between the chickenpox vaccine and the MMR (measles, mumps, and rubella) vaccine or any other live vaccine.
✅ The only routine live vaccines in the childhood schedule are chickenpox and MMR.

✅ There MUST be at least a 6-week gap between the first and second dose of chickenpox vaccine. If you are unsure of the timing of this or any other vaccines then please check with our reception team.
